New Student Pre-Registration
- Upon acceptance, the registrar mails an Admission Record and an Entrance Medical Record (available to download in Microsoft Word 6.0 format)
to be completed by the student at his/her expense. The student's personal physician or a
doctor in Family Medicine private clinics (AUH) may complete this form.
- HIP dependents may complete this form at UHS.
- A PPD screening test is done by UHS staff during the pre-registration
period.
- Completed Entrance Medical Record form with Lab results are submitted during pre-registration.
- Two days after pre-registration and PPD screening, the new student comes to UHS for PPD reading. Those with
positive results get further evaluation.
- When all above is completed, students get a medical clearance card that allows them to finish registration and use University Health Services as stipulated by HIP.
