Accident Reports

AUB employees are prone to different types of accidents such as falling down and needle prick etc. Once the accident happens, the supervisor of the unit or department head completes the Personal Accident Report form. The report includes the type of accident, the name of the injured person, and a quotation stated by the injured about how the accident has occurred. This report is sent to the UHS either

  • by the injured person if s/he needs follow up consultation or
  • by mail if the injured has been seen at the ER and does not need any further follow up.

 

Needle Prick

  1. If the accident is a needle prick and/or accident due to infections by patients or materials
  2. The supervisor of unit or head department completes the personal Accident Report form (Available at all units) within 24 hours.
  3. The supervisor or department head contacts the Infection Control Officer (ICO) at the hospital to submit the complete accident report by hand, and then sends him/her the complete accident report form by hand. In case, Infection Control officer is not available and on weekends, the nursing supervisor is called. The latter will get in touch with the UHS doctor on call as needed, ask him/her to order medications, and try to inform the ICO the next day. The UHS doctor must call the ICO the next day to ensure follow up. In case the nursing supervisor was unable to call, the supervisor or department head will reassure the exposed person that he/she shall receive the appropriate care soon. There is no need to refer individuals concerned neither to emergency room nor to UHS, pharmacy, and no need for them to leave duty. The infection Control officer will be in touch with him/her. The treatment, if any, is not urgent. It is prophylactic and can be given any time within 48 hours from exposure.
  4. The Infection Control Officer or nursing supervisor will investigate the accident, interview the employee obtain all relevant information from patient's chart and physicians. The ICO will fill the Accident Exposure Notification form, and send reports and findings to UHS by hand c/o UHS messenger. On weekends, the nursing supervisor may complete this form with the help of the UHS doctor on call.
  5. After calling UHS on weekdays or the UHS doctor on call on weekends, both the infection control officer and the nursing supervisor may order the necessary blood tests on the potential source of infection (HBsAg or HIV if necessary) and charge Hospital Administration account. A blood sample from the health care worker is drawn and held in the serology laboratory for 7 days until the accident is finalized by the ICO.
  6. Prescription may not be dispensed without doctors signature. On weekends, the UHS doctor on call may write the prescription the same day or the next morning. When UHS issues a prescription, it will be sent with a copy of the accident report (blue) to the pharmacy, which will then dispense the medication free and charge the Hospital Administration account.
  7. The UHS may request further investigations or blood tests as needed, call on the exposed individual for interview and examination and/ or prescribe treatment if required .
    *Blood tests will not be performed without evidence of completion of an accident report
  8. All procedures done at the Infirmary are documented in the Infection Control Sheet placed in the file.
